CDL/DOT Physicals - $175 Flat Fee (BY APPOINTMENT ONLY - Please call ahead to schedule)
School/Sports Physicals - $75 Flat fee (18 and under)
$160+ (18 and older)
ROTC Physicals - $50 Flat Fee
COVID-19 rapid antigen test and outside lab sent out for PCR testing is available now while supplies last. All COVID testing requires a provider consultation/visit. Please book online or call us at (928) 440-5406 for more information.
We also offer to travel to your location within 50-mile radius to do rapid COVID antigen testing for events or groups of 7 people or more. Please call or email us at manager@flaguc.com for details.

Please note that all co-payments, coinsurance, and deductible fees are due at the time of service. Our Front Desk Receptionist will give you an estimated fee rate based on your insurance plan before you are seen by our physician so there are no surprise charges.
We offer a disounted Self-Pay rate of $160-180 for a basic office vist plus additional charges for any additional testing done, medications administered or depending on the complexity of the visit.
